Pretty much what I thought it would be, 1 and a half pages of questions. Some questions are worded a bit strangely. Overall, I guess it marginally helped cure my fear of dying early. Only app of it's kind. Wish it dug in deeper -- it never asked if I skydive daily or not, ha.
