## What is New Horizons Patient Portal?

New Horizons Patient Portal is a free app that provides round-the-clock, easy, private, and safe access to your electronic health records with New Horizons Behavioral Health. You may use this app for the following functions:  
•	View your lab and other test results
•	Complete necessary paperwork before your office visit
•	Manage and view a complete list of your medicines
•	View a list of all of your allergies
•	See a snapshot of your visits at New Horizons Behavioral Health
•	Attend appointments (non-urgent)
•	Request referrals
•	Refill prescriptions
•	Check benefits
•	Update insurance or contact information
•	Complete forms
•	Ask questions through secure messaging
•	Visit summaries
•	Patient-education articles
