## What is Note & Tell?

Note & Tell helps you record pain, wellbeing, daily life, and exercises taught by healthcare professionals.

Pain can draw attention to what hurt or what you could not do. A day may also include small changes, such as walking farther, completing an exercise, finishing a task, or enjoying a positive moment.

Record pain together with sleep, fatigue, anxiety, activity, and positive experiences. Review your records yourself or during a healthcare visit.

【Record pain with daily context】

Record pain intensity from 0 to 10 as often as needed. You can also add:

• Time the pain occurred
• Pain location
• How the pain felt, such as burning, stabbing, aching, throbbing, electric, or numb
• What was happening, such as exercise, walking, stairs, or going out
• Other factors you noticed, such as weather, cold, lack of sleep, mood, or anxiety

Compare records to reflect on differences between morning and evening, after walking, or on a more comfortable day.

【Record wellbeing and what went well】

Your daily summary can include:

• Bedtime, wake-up time, and sleep duration
• Fatigue and anxiety
• Positive moments and encouraging feelings
• Concerns and difficult feelings
• Steps, walking time, and activity notes
• Falls and difficulties in daily life

Record what you were able to do and review the whole day, not only the pain score.

【Keep track of exercises you were taught】

Register instructions, repetitions, and precautions for exercises taught by a healthcare professional. Record completion and attach photos.

Review exercise completion with your other records. Note & Tell does not choose exercises or recommend repetitions, intensity, or suitability.

The Report screen organizes your latest 14 days of records for review or use during a healthcare visit.
