I was able to complete the pre-questionnaire within the app and generally found it extremely helpful as a quick reference rather than digging through emails. ### 1/5 — Can’t register *2022-09-23, version 4.16.0* Register button is disabled after entering all necessary fields ### 2/5 — Poor usability *2022-06-15, version 4.12.0* Downloaded the app 2 years ago and it was fine. I had another appointment today and had to complete a form on the app. The app doesn't work anymore, nor are there any upgrade links sent to me. Clicked app support and the page doesn't render!!
