## What is Opioid Conversion Calculator?

Opioid Conversion Calculator is built for health professionals who need a clear, efficient way to compare opioid doses and review the clinical context behind a conversion.

CALCULATE AND COMPARE

- Calculate total oral morphine equivalent (OME) across multiple opioids and routes.
- Review calculated target doses after applying a chosen Target OME percentage.
- View conservative, rounded-down target and breakthrough dose suggestions.
- Move quickly between the calculation, individual results and supporting information.

REVIEW CLINICAL INFORMATION

- Read practical points alongside formulation-specific pharmacology.
- Switch between supported formulations without leaving the result.
- Follow links to the supporting references.
- View official products and strengths for supported clinical regions, independently from the app language.

TAILOR THE WORKFLOW

- Edit conversion factors and choose which opioids appear with Pro.
- Export and import settings when moving between devices.
- Use the app in English, French, German, Italian, Portuguese (Brazil) or Spanish.

PRO ACCESS

The core calculation remains available without creating an account. Pro adds professional detail and customisation. Available plans, prices and any introductory offer are shown using the information supplied by the App Store before purchase. Purchases and subscriptions can be restored in the app.

CLINICAL USE

Equianalgesic conversions are estimates and do not replace clinical judgement. Consider incomplete cross-tolerance, patient factors, formulation, route, local guidance and monitoring when interpreting a result. Review the references and terms of use before using the calculator in clinical practice.

### 1/5 — In accurate conversions.

I downloaded the app as a alternative to the old CDC guideline app, which is no longer available for download. I ran it through a couple of calculations. It calculated that hydromorphone 4 mg = 20 MME. This is incorrect as the answer should have been 16 MME. Also, oxymorphone is missing from the conversion list. It is still a medication and it does still exist. There should be a tool for calculating its conversion. Very poorly researched, very misleading.

**Developer response:** Thank you for your feedback. I’d like to clarify that the app is based on extensive research, drawing from dozens of papers, expert opinions, and various international guidelines. While I understand that locally in the United States a 4:1 hydromorphone-to-MME conversion might be common, most global guidelines, including those recommended by many experts, suggest a 5:1 ratio, which is what the app follows. It's important to note that opioid conversion is not an exact science, and each patient needs to be considered individually, with adjustments made based on clinical judgment.

As for oxymorphone, you're absolutely right—it’s still in use, and we can certainly look into adding it. We genuinely appreciate feedback that helps us improve, and we encourage users to share suggestions through our in-app feedback portal so we can address concerns directly and continue making this the best resource possible. A 1-star review is disheartening, but we are committed to improving based on your insights.

Thanks again for your input.
