Palliativ-Portal Palliativmedizin – Hospizversorgung Das Palliativ-Portal ist speziell für palliativmedizinisch/-pflegerisch und hospizlich betroffene Menschen und Ihre Angehörige, Familie und Freunde, entwickelt worden, um in kritischen Phasen des Lebens ausreichende Unterstützung zu finden. Wichtig ist es hierbei eine Hilfestellung zu geben, wenn es darum geht, Palliativmediziner, -stationen oder ambulante Pflegedienste oder SAPV-Teams mit Palliativfachkompetenz sowie Hospizvereine und stationäre Hospize zu finden, die in der Lage sind, kompetent an Ihrer Seite zu sein, um Sie zu begleiten. Auch Pädiatrische Palliativteams (SAPPV) können Sie im Palliativ-Portal finden. Nach den Definitionen der Weltgesundheitsorganisation (WHO) und der Deutschen Gesellschaft für Palliativmedizin (DGP) dient die Palliativmedizin der aktiven, ganzheitlichen Behandlung von Menschen mit einer fortschreitenden oder fortgeschrittenen lebensverkürzenden Erkrankung.
