اینجانبان با علم به مسولیت کیفری شهادت دروغ ،نسبت به موضوع و اطلاع داشتن از حرمت شرعی و مجازاتی که در قانون برای شهادت دروغین در نظر گرفته شده است، گواهی می دهیم ، مورخه ۱۴۰۳/۱۲/۱۵ ساعت …… به نشانی سنندج ، بلوارشبلی ، کوچه قضات ، پلاک ۲۰ مشغول به انجام کارهای ساختمانی ، دو نفر از کارگران در ساختمان حضور داشته و مابقی اطراف ساختمان به حرفه ی خود کارگمار بودیم ، ماموران محترم با لباس شخصی که تعداد آنها سه نفر بوده بدون اجازه وارد ساختمان ، که به کرات خواستار علت حضور ، مدارک قانونی جهت تجویز و دستور مقام محترم قضایی برای حضور و تردد آنها در ساختمان شده که علی الاسف ، علاوه بر عدم ارائه مدارک قانونی با مخالفت و خصومت آنها مواجه شده ، لازم به ذکر است مشارالیها کل ساختمان ……. طبقه را که  حدودا ۸۰ درصد آن تکمیل شده ، را سیر کردند .
مضافا اینجانبان علاوه بر امضا و تایید استشهادیه فوق الذکر ، آمادگی کامل جهت حضور نزد مقام قضایی محترم و ادای شهادت میباشیم .
