The Personal Health DashboardTM (PHD) app provides access to your organization's well-being program and is available exclusively to eligible US HealthCenter users. Contact your organization or well-being program sponsor to check if your organization offers US HealthCenter services.


The PHD app includes all the core features of the website, allowing you to:



- Complete health assessments
- Evaluate your risk for over 75 health conditions, including cancer, heart disease, diabetes, and depression
- Receive a personal health report card
- View your Health Scores and WellCentive Score
- Receive personalized health improvement recommendations
- Track steps, vitals, water intake, sleep, and more
- Join challenges
- Easily connect to links to all of your company benefits such as Telemedicine , EAP, Health Insurance and more
- Participate in behavior change modules for Stress Reduction, Diet and Nutrition, Fitness, and Tobacco Cessation
- Connect with a variety of wearables
- Purchase discounted wellness products
- Explore health topics through an extensive health and video library
- Participate in improvement Surveys
