## What is POTS Check?

POTS Check is a free home wellness screening tool that helps you explore whether you may have Postural Orthostatic Tachycardia Syndrome (POTS), or other forms of orthostatic intolerance that cause symptoms like dizziness, rapid heartbeat, and fatigue when standing.

POTS Check helps you document your cardiovascular response and arrive at your provider appointment with organized, clinically-legible data.

**HOW IT WORKS**

Using your Apple Watch and iPhone, POTS Check guides you through the NASA Lean Test, a clinically-recognized screening protocol that monitors your heart rate response when transitioning from lying down to standing.

Your Apple Watch continuously monitors your heart rate throughout the test. If you have a blood pressure cuff, you'll enter readings at key intervals — BP data adds critical context, including Proportional Pulse Pressure (PPP), which can reveal vascular patterns that heart rate alone cannot detect.

After the test, you receive a clinical-format report with graphs and a per-minute data table you can bring to your healthcare provider for interpretation.

### 1.2.1 — 2026-07-22

This update fixes an important reliability bug reported by one of our users. Thank you for the bug reports, they make the app better for everyone.

- Fixed a bug where a completed test could fail to save if your iPhone was locked while your Apple Watch delivered the results. Your Watch now keeps its own copy of every test until your iPhone confirms the result is safely saved, and saving retries automatically after you unlock.

- Fixed a blank screen that could appear after adding notes at the end of a test. If your result hasn't arrived from the Watch yet, you'll now see a clear status screen with a "Sync from Watch" button instead.

- Notes added right after a test are now reliably attached to your results, even if the result is still syncing.

- Fixed formatting issues when sending feedback through third-party email apps like Gmail.

- If saved tests ever can't be read, the app now tells you instead of quietly showing an empty history.

- Cleaned up the last places in the app that still showed our old working name, POTS Screener.

**Version 1.2 — Clinical Alignment Update**

This update incorporates direct feedback from the Bateman Horne Center (BHC), a leading ME/CFS and orthostatic intolerance clinic, to improve clinical accuracy and provider usability.

**Protocol Updates**

• Extended baseline duration: The default supine baseline is now 10 minutes — matching BHC's recommended clinical minimum — up from 5 minutes. Choose 5, 10, or 15 minutes to suit your needs. A 5-minute selection now shows a clear advisory about reduced accuracy.

• Blood pressure as the primary protocol path: The test now opens with a BP cuff confirmation step. BP is the recommended protocol path; if you don't have a cuff, the app clearly notes your results are limited in scope and unsuitable for sharing without BP data.

• Proportional Pulse Pressure (PPP): A new vascular metric calculated from your standing BP readings. PPP can reveal "failure to constrict" on standing — a key pattern invisible from heart rate alone. Your lowest observed PPP and the minute it occurred appear in results alongside a clinical reference range.

• Orthostatic Hypertension detection corrected to use SBP-only criteria (AHA definition).

**Results & Reports**

• Provider-format PDF report now includes a per-minute clinical data table (HR, SBP, DBP, PPP at each standing minute), matching the format used in specialist clinic presentations.

• Verdict language removed: The app no longer renders a diagnostic-style conclusion. Results present your data with established reference ranges and explicit provider-interpretation framing throughout. Blood pressure flags (Orthostatic Hypotension, Orthostatic Hypertension) remain, with explicit AHA source citations.

**New Feature: Test Notes**

Add free-text notes to any test session — current symptoms, medications, recent activity. Notes appear in results and in the PDF report.

**Education & Guidance**

• Orthostatic intolerance framing: POTS is just one form of OI. The app now explains the broader context of orthostatic intolerance from first launch through post-test results, informed by BHC clinical guidance.

• Pre-test positioning guide: An expandable NASA Lean Test card with visual guidance (buttocks away from wall, knees soft) added to the pre-test checklist.

• Pre-test environment instructions updated: quiet, low-lit space, no stimulation, helper encouraged for BP readings.

• Recovery guidance expanded: The post-test phase now includes instructions to lie down, elevate feet, drink water, and eat a light snack.

• Expanded Learn section with OI, orthostatic hypotension, and vascular response content.
