The app is terrible. It barely gets the job done, regular glitches. If you have no choice go with this, otherwise a little research might do your business good and keep them in compliance with local policies

Tried several times to submit my questionnaire for work - answered all questions and the submission went nowhere. Had to switch to a browser to have my responses saved. This was only helpful to upload pics of the vaccine card but even that was far more difficult and slow than it had to be.

This app has worked great in me being able to provide proof of my COVID-19 vaccination status to my employer. In the beginning there were many challenges getting my information added/updated, but application support has been phenomenal in helping me resolve any issues I’ve experienced, up to and including today. Yesterday, I was not able to add my 2nd booster shot to my current vaccination status in the app. So I emailed application support, and within 24 hours they were able to resolve my reported problem. As a result, I cannot recommend this app enough, and their support is simply 5 stars ⭐️ ⭐️⭐️⭐️⭐️ stellar!!! 👍

This app has been very helpful with managing our employees covid info (vaccine records, test results, etc). Our HR team loves it and employees have had no issues or problems - it is very user friendly. Highly recommend.

This App offers a simple approach for reporting your health conditions to your employer. The questionnaire is designed to be complaint with CDC guidelines, and also is concise and expressive, supporting the logic conditions among the questions in the questionnaire.  All in all, this App can meet  the needs of your employer in this pandemic age. Thanks!
