## What is PrismMedConnect?

PrismMedConnect is the official mobile app by Acentus, a Prism Medical company - connecting patients and account teams with convenient access to Continuous Glucose Monitoring (CGM) orders, referrals, and care-related information in one place.

The app supports two types of users:

Patients
• View CGM order history and account information
• Submit new CGM supply orders
• Track order and delivery status
• Access important updates related to your care and supplies

Account Managers and Sales Representatives
• Manage patient referrals and CGM orders
• Monitor key performance indicators (KPIs) and activity dashboards
• Scan and manage supporting documentation
• Access tools that streamline patient service and workflow management

PrismMedConnect is designed to simplify communication, improve visibility into the ordering process, and create a more efficient experience for both patients and care teams.
