## What is ProcVault?

Your residency case log ended the day you graduated. Everything since — as an attending, a locum, an NP or PA — is yours to document. And every new hospital, every privileging renewal, every FPPE review asks the same thing: how many, how recently, and who signed off.

No system tracks that. ProcVault does. Supervisors sign on your phone at the bedside, it stays on your device, and it goes where you go.

For attendings credentialing at a new hospital, locums clinicians who privilege repeatedly, NPs and PAs asked for numbers, nurses keeping their own competency proof, and residents who want a copy that outlasts the program's software.

SIGN-OFFS AT THE BEDSIDE, NOT WEEKS LATER
• Add the supervising clinician to any entry, then hand them your phone — they sign on the screen, like a delivery signature
• Before signing, they confirm the standard competency elements: indication, consent, identifiers, landmarks, sterile technique, complications, placement, documentation
• Only what they tick is recorded — nothing is pre-checked or assumed
• Every signature is timestamped and bound to that procedure
• Signed entries lock — no editing after attestation
• Supervisor walked away? Your dashboard keeps an "awaiting signature" list

LOG IN TWO TAPS
• Common procedures on one grid, 60+ more in search, custom entries for anything else — your frequent ones surface automatically
• Role (performed / supervised / assisted), date, site
• Technique tracked the way committees ask: ultrasound guidance where it applies, video laryngoscopy for airway

YOUR FORMS, FILLED AND KEPT (PRO)
• Import your facility's form or scan the paper copy with the camera
• If it's a fillable PDF, ProcVault fills the form's own fields — your details, the signing date, signatures — exactly where the form declares, and leaves patient and evaluator boxes blank
• Auto-Fill adds your name, unit, and title beside the labels it reads, and fills the evaluator table from the supervisors who signed your entries — initials, printed name, captured signature
• Tap any blank to type. Export a flattened, print-ready PDF
• Completed forms save to your vault to produce at the next site

PRINTABLE PROOF, BUILT IN (PRO)
• Attestation PDFs: summary counts, technique breakdown, the detailed log, the confirmed competency elements, every individual supervisor signature, and final signature lines
• Standards-aligned templates ready to print: an ultrasound-guided IV competency record, an ultrasound credentialing tally aligned with published guideline minimums, and a per-procedure competency validation for FPPE-style review

ALSO IN PRO
• PHI-safe image attachments: photos taken in the app never reach your camera roll, and on-device scanning proposes black-box redactions you review before saving

SEE WHERE YOU STAND
• Dashboard totals, monthly counts, per-procedure breakdowns — every number tappable through to the entries behind it
• Set credentialing targets and watch the bars fill
• Free CSV export of your complete log, signatures included

## Version history (last 5 releases)

### 1.2.2 — 2026-10-05

• Take Photo: attach a picture with the camera inside ProcVault. It goes straight to redaction and never lands in your photo library.
• Simpler pricing: ProcVault Pro is now $24.99 once, or $9.99 a year. Existing subscriptions keep working.
• CSV export of your full log is now free for everyone.
• Self-attested entries are never presented as supervisor sign-offs — on attestation PDFs, competency templates, or a form's evaluator table.
• Fillable forms no longer get your name, initials, or today's date in patient, evaluator, or verifier boxes.
• Radio-button answers on fillable forms (such as Met / Not Met) now fill correctly, and each signature box gets its own signature.
• Deleting a signed entry or removing a signature now asks first.
• ProcVault Record asks which record you want instead of guessing.
• Before a form is kept in your vault, you're reminded to check it for patient identifiers.
• Long log lines wrap on PDFs instead of running off the page.

### 1.2.1 — 2026-09-24

Form filling is now precise or it doesn't happen. ProcVault fills a form's
own fields when the PDF declares them, so values land exactly where the
form says they belong — and it no longer marks competency boxes on forms it
can't read reliably.

Supervisors now confirm the standard competency elements before signing —
indication, consent, identifiers, landmarks, sterile technique, and more.
Only what they tick is recorded.

Signing your own procedure is recorded and labeled as a self-attestation,
and a sideways signature can be turned upright before saving.

### 1.2.0 — 2026-08-26

Review & Sign — import a hospital competency form and ProcVault reads
the form's own checklist items, so your supervisor confirms exactly
what's printed on the page, then signs it right in the app.

Marks now land in the correct MET and INITIALS cells on ruled forms.

On-device intelligence separates real competency items from headers
and instructions. Nothing ever leaves your device.

### 1.1 — 2026-08-15

Polish update:
• Signatures now fill their space properly on entries, attestation PDFs, and hospital forms — no more tiny marks floating in a corner.

Thanks for the early feedback — keep it coming via Profile → Request a procedure.

### 1.0 — 2026-08-10
