The area to enter your date of birth isn’t visible, you are aimlessly scrolling at a blank screen. There is no option to manually enter. Also in the Financial Assistance area, I continued to get an error message when trying to enter my Pharmacy. I had to select that I was not working with a pharmacy just to move forward.
