**Developer response:** 操作手順がわかりにくくて大変申し訳ございません。
操作対象のPDFを開いたのちに、結合したいファイルを選択するようになっております。
そのため、ファイルAを開いたあとで「結合」のメニューを選んだら、結合したいファイルとしてファイルBのみを選択していただければ、最初に開いたファイルAにファイルBが結合されます。
お試しください、よろしくお願い申し上げます。

また、申し訳ございませんが結合方向は指定ができませんので、
一旦結合した後で、別の編集としてページの入れ替え作業等を行ってください。
お手数をおかけしますが、よろしくお願いします。

文字入力時に濁点が入力できなとのことですが、それはファイル保存時のファイル名入力時でしょうか？

こちらで現象が再現できるか確認し、対策を検討いたします。
iOSは15.5でよろしいでしょうか？

応急的な処置としては、音声入力で入力されるか、一旦仮のファイル名で保存し、保存後にiPhoneのファイルの機能で名前を変更してみることもお試しいただければと思います。
