## What is Survey Alert by SignalCoreIQ?

Survey Alert helps nursing home operators monitor recent survey activity, complaint activity, citations, and nearby operator intelligence in one focused, sign-in-only app.

Use Survey Alert to review recent activity across your available access area, filter by 30, 60, or 90 days, search by region or state, and open facility detail directly from the overview or map. Facility records can show standard survey history, citation counts, citation groups, highest citation detail when available, and complaint activity.

Operator Intel keeps Scout Reports and Operator Questions separate from exact map pins. Reports and questions are anonymous, moderated, and designed to help operators understand nearby activity without exposing precise reporter identity or location.
