Svastir.Care enables patients to take charge of their health. They can monitor their day-to-day activities, request care, keep track of their vital readings like blood pressure, heart rate etc., request assistance from their care community, manage their medications and connect to the care provider network through secure multi-party video calls. Physicians, home care companies, and care managers around the world use the mobile functionality of our app to keep an eye on their patients, monitor them, keep them engaged and always be available through telehealth, messaging, and instant video calls. Our App is an extension of our web platform, bringing mobile functionality into your care initiatives.

Vitals-

◦ Your physician monitors your readings and will adjust the care plan accordingly

Request Care-
◦ Ask your care community for help with transport, supplies (like diabetic strips, wound care supplies), an appointment with your physician or nurse (virtual video call), services (help with laundry, groceries or cleaning), tech support or request a medication refill.

◦ View all your documents that you have uploaded on the system.
