## What is TakeChargeApp?

This app will provide personalized reminders of provider prescribed medicine and exercises.  It aims to help users remember these important parts of their treatment while out of the hospital. The doctor will sign in and prescribe the medications and the exercises to the patient. The patient will be able to see those medications and keep track on them.
