When I select next after filling the form, the app closes itself. What should I do? Fix the issue fast.

The app keeps closing itself, I just downloaded and I trued to open it but it keeps of closing itself

The app keeps closes by it self whiteout even opening. May it has to do something with the apple. But, it is going really inconvenient at this time. Hope concerned party puts their attention into this matter soon.
