Trellus Health, and the Trellus Health Provider app make it easier to support and empower your patients to self-manage in between office visits.  Through the Trellus Health Provider app, connected providers can partner with a dedicated team of resilience coaches, educators and dietitians trained to work with your patients to navigate their personalized resilience roadmap to effective self-management and help you address their physical, mental and social well-being all while supporting your plan of care.

Trellus Health is an evidence based, self-management solution founded by Dr. Marla Dubinsky and Dr. Laurie Keefer from the Icahn Schol of Medicine, Mount Sinai New York.  Trellus Health members gain access to expert driven disease monitoring and receive personalized self-management training and support in between office visits without disrupting your workflow.  We provide your patients with the problem-solving skills, tools and education supporting patient care plan adherence with unlimited resilience coaching and nutritional support, reducing burden on providers and their staff.
