After two questionnaires on first two occasions, no other questionnaires are forthcoming. ### 1/5 — Doesn’t work *2026-04-29* Same problem that others reported. Reported it to my VSO, but no fix yet. No change in its application. No feedback mechanism. Strongly advise talking with your provider about a different mechanism as this one is broken and likely not serviced. ### 1/5 — Pure Trash *2025-11-06* I registered like I was supposed to, the app has only allowed me to fill out the forms 2 times, it keeps resetting the date for the next date & wont let you fill out the one for that day.
