Full Patient Clinical Record (PCR) — completely rebuilt

All 12 sections now mirror the web back office exactly: ambulance & crew details, patient identity, next of kin, past medical history, initial assessment with mechanism and COVID screening flags, a full observations timeline with timestamped repeated obs sets, Glasgow Coma Scale scoring, capacity & consent with digital signature capture, secondary assessment with body map, IV cannulation, medical management with Entonox/Penthrox, outcome & transport, patient declined/refused treatment declaration, and treated/seen-by clinician sign-off with signatures.
