## What is Welby?

Welby is a chronic care management app that brings together your Apple Health readings (heart rate, blood pressure, weight, blood glucose, oxygen saturation), the medical records you've connected from your providers through Apple Health Records (medications, allergies, conditions, lab results, immunizations), and your day-to-day check-ins into one place your assigned Welby care team can review.

Welby reads data from Apple Health that you have explicitly authorized in iOS. The app itself does not pair with or operate medical devices.
