## What is Y Tế HCM?

Ứng dụng khai báo y tế điện tử của Sở Y tế TP. Hồ Chí Minh cho phép người dân khai báo thông tin y tế thay thế cho hình thức khai báo giấy tại các cơ sở y tế trên địa bàn thành phố. 

Ngày 10/7/2021, Sở Y tế công bố triển khai trả kết quả xét nghiệm bằng QR code trên hệ thống khai báo y tế điện tử dành cho người dân có nhu cầu cấp giấy xác nhận kết quả xét nghiệm âm tính với virus SARS-CoV-2. 

Danh sách các cơ sở y tế đủ điều kiện xét nghiệm cho người dân khi có nhu cầu ra khỏi thành phố (cập nhật đến 17g30 ngày 08/7/2021) được công bố công khai trên Cổng thông tin điện từ Ngành y tế tại: 
http://www.medinet.hochiminhcity.gov.vn/quan-ly-chat-luong-kham-chua-benh/cac-benh-vien-trung-tam-y-te-du-dieu-kien-xet-nghiem-cho-nguoi-dan-khi-co-nhu-c-c8-47739.aspx.

Để kịp thời triển khai công tác hỗ trợ chăm sóc sức khỏe cho các bệnh nhân F0 cách ly tại nhà, ngày 17/8/2021, Sở y tế Thành phố ban hành hướng dẫn tại Cổng thông tin ngành Y tế. Ứng dụng Y tế HCM đã cập nhật tính năng hỗ trợ cho người dân tiếp cận liên lạc với tổ phản ứng nhanh tại cơ sở và cho phép khai báo về tình trạng sức khỏe khi cách ly tại nhà.
