## What is Yellow Card?

Use the Yellow Card app to report suspected safety concerns involving healthcare products, including a side effect with a medicine or an adverse medical device incident.

The Yellow Card scheme is the UK system run by the MHRA* for collecting and monitoring information on suspected adverse reactions to all medicines including vaccines, blood factors and immunoglobulins, herbal medicines and homeopathic remedies, and all medical devices available on the UK market.

The Yellow Card app allows users to:

Report safety concerns about:

> Suspected side effects to a medicine or vaccine (including herbal products and homeopathic remedies)

> Adverse incidents involving a medical device (including software, apps and artificial intelligence)

> Suspected defective medicines (a product that is not working properly or not to the manufacturer’s specifications)

> Fake and/or unauthorised medicines

> Blood products

Create an account:

> View previously submitted reports

> Update your reports

> Create a watchlist for alerts to products of interest

> Track new safety information published by the MHRA

> View numbers of reports received by the MHRA

All reports received through the app or website can help the MHRA identify potential new concerns about a medicine, vaccine or medical device and act as an early warning for further investigation. The MHRA will review the product if necessary, and take action to minimise risk and maximise benefit to the patients.

*The Medicines and Healthcare products Regulatory Agency (MHRA) is an executive Agency of the Department of Health and Social Care; the MHRA protects and promotes public health and patient safety by ensuring that medicines, healthcare products and medical equipment are used safely and meet appropriate standards of safety, quality, performance and effectiveness.

### 1/5 — Takes all the side effect details but won’t allow saving/submitting

*2025-10-08*

Firstly, this app is pretty much unusable as is. There isn’t much point in a patient using it.

When I went to report a side effect, it allowed me to get all the way through to the written side effects, but did not allow me to save the report, leading to frustration.

Some of the fields, e.g. date stopped taking, if you click on it by accident, you MUST enter a date when you stopped taking the medication, even if you are still taking it. So although this is done for data quality purposes, it doesn’t give an accurate version of the truth.

### 1/5 — App is up usable in its present form

*2025-07-20*

I read the reviews and thought I’d give it a go anyway, I can’t even create an account to raise concerns about a drug I’ve been prescribed by my Doctor, it’s for Diabetes but in actuality it is this new weight loss drug. Based on my experience, how this drug ever got to be recommended by the UK Government is behind me. The app would be good to be able to inform the people that need to know about side effects of various drugs, but if you can’t even create an account, why bother developing the app! Fix it or remove it, because at present it’s not fit for purpose.

### 2/5 — Hangs before Submit

*2024-12-22*

Tried this app to report something simple - but the app hangs when filling in the text of the description - screen goes grey all over - but text still editable. It is saved however in pending and I eventually sent it. This nerds sorting whatever is causing it to happen. Poor usability reports go back 3 years at least so not convinced it’s ever going to get fixed. At least it worked in the end. The website is apparently even more problematic - although that report is now old.

### 1/5 — Now non-functional

*2024-11-10*

This app is vital but it needs at least an update to enable it to work on current operating systems. At the time of writing (v 24.1.5) it just stalls and you cannot submit any reports.

### 1/5 — Can’t report anything as keyboard doesn’t disappear

*2024-10-27, version 24.1.5*

Can’t report anything as keyboard doesn’t disappear

### 1/5 — App crashes

*2024-10-23*

Tried to report a side effect and it crashed during free text input. Come on developers. This is just not good enough for such an important bit of software.

### 1/5 — Too buggy

*2024-09-14*

The app has crashed twice now when I’ve tried to write in the free text box on symptoms. I’ve been unable to complete a report.

### 1/5 — Selena Allwood

*2024-08-03*

Downloaded to report a side effect on a new medication. Trying to be helpful. App doesn’t work. Very frustrating. Rubbish. Sorry but I think this doesn’t work on Purpose

### 1/5 — Waste of time

*2024-08-02, version 24.1.5*

Tried to report side effects of a statin but after 20 mins of entering details it seemed to freeze. What a waste of time😕

### 1/5 — Useless

*2024-07-24, version 24.1.5*

Spent ages filling in the information and then the app froze so can’t save. A frustrating waste of time. Worrying that important information on medications is not being captured as a result.
