It has stream lined my transition of care from Hospital to SNF and Dialysis units. I also use it to continue treatments in the clinic that were started in the hospital. The option to have social workers and case managers on board really help getting patients assigned to dialysis units and post hospital clinic appointments. My group has three inpatient physicians, one NP, one out patient physician, and an office manager. We can all communicate at the same time regarding a patient without me having to make several phone calls. Overall I’d say this app has greatly improved the care we provide.
