The clearest difference is rating: MILKLAB at 4.9 against Alpaca Peruvian Chicken's 4.0. MILKLAB also leads on ads (none vs shown) and monetization model (free vs free with in-app purchases). Alpaca Peruvian Chicken's advantage is review sentiment (30% vs 0%) and update cadence (every 2 months vs every 4 months). On price, ratings volume, in-app purchases and device support there is nothing between them.

Both are free to download. Neither carries in-app purchases, so what you see is what you pay.

MILKLAB holds the better App Store score, 4.9 against 4.0. Both are backed by a comparable volume of ratings — 187 and 193 respectively.
