SymptomLog is a private daily diary for one person living with one long-term condition. Log how the day actually went in under a minute, then show your doctor a record instead of trying to remember. At setup you choose your condition — migraine, IBS and gut health, autoimmune, fibromyalgia, chronic pain, endometriosis, allergies, mental health, or something else — and your starting symptom list is built around it. WHAT ONE DAY LOOKS LIKE Tap the quick-log card on the home screen. Pick one of five moods, from Terrible to Great. Slide each symptom you track from 0 to 10. Tap anything that may have played a part: poor sleep, stress, weather change, a certain food, alcohol, exercise, a missed dose, menstrual cycle, dehydration, overexertion, travel, illness. Add a note if you want one, then save. That's the whole thing. The home screen shows your logging streak, your total days logged, and a week-at-a-glance strip, so you can see which days you missed and go back and fill them in. An optional reminder at a time you choose nudges you once a day. IT STAYS ON YOUR PHONE There's no account, no sign-up and no cloud sync. Your entries are stored on this device — no copy sits on a server, and nobody else can open your log. PREMIUM: TURN THE LOG INTO AN ANSWER • See the shape of it — trend lines, a severity breakdown, a calendar heatmap and your mood distribution across 7, 30 or 90 days. • Check your triggers — a frequency chart showing which triggers you tapped most over the period you chose. • Hand your doctor something readable — a full Doctor Report as a PDF, a single-symptom report, or a side-by-side comparison of two date ranges. • Keep the raw numbers — export everything as a CSV file. • Track what you actually have — add, remove and reorder your own symptoms. • Look back a full year instead of 30 days. You get a few minutes with your doctor. Spend them on the conversation, not on reconstructing the last three months out loud. GETTING STARTED Setup takes about a minute: choose your condition, choose the symptoms you want to start with, set a reminder if you'd like one. Daily logging and the last 30 days of history are included at no cost. Premium is available weekly or yearly, or as a one-time Lifetime purchase, whenever the charts and reports are worth it to you. The weekly and yearly plans are auto-renewing subscriptions, charged to your Apple ID at confirmation of purchase and renewing unless canceled at least 24 hours before the current period ends. Manage or cancel anytime in your account settings. A trial is offered on a plan only when your account is eligible for one. Lifetime is a single purchase with no recurring charge. Current prices are shown on the plan screen in your local currency. Track chronic illness without turning your health into homework. Tended is a privacy-first health journal and symptom tracker for people living with long-term symptoms, pain, fatigue, flares, and appointments that never quite fit into ten minutes. Log in seconds, notice possible patterns, and prepare for doctor appointments without streaks, guilt, accounts, analytics, data collection, or external servers. LOG IN SECONDS, EVEN ON HARD DAYS When thinking is hard, Tended keeps logging light. Record a day rating, symptoms, pain, medications, energy, sleep, mood, bowel changes, food, triggers, activities, periods, photos, and notes at your own pace. Use Apple Watch, widgets, Siri, the Lock Screen, or the full app when opening your phone feels like too much. Duplicate yesterday, edit only what changed, or describe your day in your own words with optional on-device AI. BUILT FOR CHRONIC ILLNESS Tended is made for fluctuating conditions where a normal habit tracker does not fit. Use it as a chronic illness journal, symptom tracker, pain tracker, medication tracker, flare tracker, energy tracking tool, sleep tracking companion, and body map. Condition starter sets help you begin with relevant symptoms and triggers for fibromyalgia, ME/CFS, lupus, endometriosis, PMOS / PCOS, POTS, EDS, migraine, MS, IBD, PMDD, and many more. SEE PATTERNS IN YOUR OWN DATA Your journal can become more useful over time. Tended may help you notice possible links between symptoms, medications, food, sleep, activity, weather, Apple Health averages, menstrual cycle phases, flares, and better days. Insights are based on what you log yourself. They are self-reported observations, not clinical findings, so you stay in charge of what feels useful. PREPARE FOR APPOINTMENTS When brain fog arrives before your appointment, Tended helps you bring the important parts with you. Use Doctor Mode, appointment notes, body map history, medication adherence, symptom trends, focused visual reports, and PDF health reports to explain what has been happening. Tended+ unlocks longer history, deeper pattern insights, trigger experiments, appointment-ready PDF health reports, focused evidence and care PDFs, and CSV export for people who want to go deeper. STAY CONNECTED, WITHOUT OVERSHARING If you choose, Tended can help you share small practical updates with someone you trust through Supported, the carer companion app. Send a quiet check-in, a quick need, or a regular summary without exposing your whole private journal. FREE FOREVER. NO GUILT. Every tracking feature is free and unlimited. There are no streaks, no shame, and no "you missed a day" messages. Tended+ is for people who want longer-term insight and appointment-ready reports. The free journal stays useful on its own. PRIVATE BY DESIGN No account. No analytics. No data collection. No external server. Your journal lives on your device or in your personal iCloud. Apple Health data is read only when you allow it. Optional on-device AI features run on your device where available; your journal is not sent to Cocoacorn servers. Use Tended gently, at your own pace. Bring what helps. Leave the rest. MEDICAL DISCLAIMER Tended is a personal health journal, not a medical device or clinical tool. It is not a substitute for medical advice or emergency support. Content is based on what you log yourself and may be incomplete or inaccurate. Always consult a qualified healthcare professional for medical decisions. Tended does not monitor journal entries for distress. If you are in crisis, contact local emergency or crisis support. Verdict

The call

The clearest difference is rating: Symptom Log at 5.0 against Tended's 4.3. Symptom Log also leads on iOS requirement (15.0 vs 18.0). Tended's advantage is update cadence (every 5 days vs every 2 weeks) and ratings volume (7 vs 1). On price, ads, in-app purchases and monetization model there is nothing between them.

CostPrice · In-app purchases · Ads · Monetization

Both are free to download. Symptom Log sells 3 in-app purchases, starting at $2.99 per 7 days and Tended sells 1 one-off in-app purchase. Whichever you choose, the download price is not the number that matters.

ReceptionRating · Positive reviews · Number of ratings

Symptom Log holds the better App Store score, 5.0 against 4.3. That said, the two scores are not equally well evidenced: Tended has 7 ratings behind it against 1, so its average is the more settled of the two.

UpkeepUpdate frequency

Symptom Log ships an update every 2 weeks, Tended every 5 days. The most recent releases landed on October 10, 2026 and October 19, 2026 respectively.
