Docket® partners with public health heroes to make it easier than ever for you to access personal and family immunization records directly from Alaska Department of Health, Idaho Department of Health and Welfare, Minnesota Department of Health, New Jersey Department of Health, Utah Department of Health and Human Services, and Wyoming Department of Health. Track upcoming shots, review past immunizations, and share your official immunization reports. Alaska, Idaho, New Jersey, Utah, and Wyoming residents can access a SMART® Health Card vaccine credential QR code to facilitate travel to places with COVID-19 restrictions. Please stay tuned for announcements from other states. For more information about SMART® Health Cards, visit https://smarthealth.cards. For more information about the Vaccination Credential Initiative, visit https://vci.org. NOTE: You must have a valid email address and/or phone number on file with your state's immunization registry in order to access personal or family immunization records using this app. Please consult with your health department if you have trouble locating your records using this service.   • Easy to use!

    I never knew there was an easy way to access my sons immunization records from his entire life other than going and simply requesting records from multiple providers until I ran across this convenient app one day while doing some research. It is so convenient when needing to provide my daycare or school vaccine records. There is a way to just download all the vaccine history and administration dates in pdf format to print and is very user friendly. You won’t regret giving it a try. It works for adults too!!
  • Good for portablility but….

    …. Doesnt update very quickly. I got my 3rd shot several days ago but it apparently hasnt worked its way thru the system because it isnt showing on the app. So dont lose your card. Youre still gonna need it.

    **Edit** it is now more than 10 days and still not updated. Also, you cant clear old searches. My OCD doesnt like outdated (useless) info on my screen.

    **Edit #2** had to delete the app and reinstall it for that shot to show up. Seems to be an ongoing bug because edit #2 is more than a year later (shot #6) and still having same problem with the 2023 booster.

    Hi there - Thank you for reaching out. Please note that Docket reflects what your provider or pharmacist report to your state's immunization registry (AKA "IIS"). Once your IIS record is updated accurately with your correct dose information, you can refresh Docket to see your new shot(s). Alternatively, you can also reach out to your state directly.   • Your Community Pharmacist

    I recommend this to the patients I give vaccines to at my pharmacy. It makes my life easier as a pharmacist who deals with a very busy and short staffed pharmacy. Thanks to the ones who continue to provide a valuable, and reliable app for healthcare providers and patients.
    -It would be very helpful if the app would allow for provider access, but I’m not sure that’s legally feasible.
    -You should consider connecting with APhA to see if you can promote a this helpful app/tool for pharmacists. If there were more state databases available to view in the app, it would also allow patients from outside of the state to provide documentation to pharmacists who may not be able to verify vaccine documentation without it.

    TLDR;

    10/10 would recomend. 100%. Just needs more access to additional state databases which helps improve patient care and patient safety.
