Don’t need a drop down for State. Putting two letters in the box is faster, better and easier than scrolling a list.
Every other form I fill out has my information pop up. I just need to click on the information. Yours, needs a lot of typing.
The same information is needed multiple times. Other forms have a box to check “Billing address is the same as mailing address”

Could use some improvements
New to this app...needs some improvement. Sone exsmples:
* creating fields a little bigger to click to enter information
* ability to enter more than 1 provider per form for records release
* saving certain information (name, birthday, address, phone number) because they don't normally change ( but can edit and save if needed).
* always sending copies of completed forms. It asked if i wanted one today but did not for the other 2Show lessdrewgb

Our situation
Own our home, monthly expenditures $5,200 Receive $1,000 a month for service connected disability ( Hearing loss) no Car note. My wife is unable to do basic things such as cook or bathe needs help with meds. She was hospitalized in Dec for 30 days. She is in able to go to our bedroom on 2 nd floor. Wiped my 8-page document
When I opened the app, it had me put in my email and my name. Normal enough. I then spent 30 minutes filling out an 8-page medical document. When I hit submit, it told me I had to put in my name and email once again. Okay a little weird but no big deal. When I did that, it did not submit my document but instead brought me back to a COMPLETELY WIPED version of the 8-page document that I had just filled out. Only one flaw…
I love the convenience of this app. It really saves time and a whole lot of hassle. However, every time I fill one out it will not allow me to check the box on the first line. I am repeatedly told that I have tapped outside the required field. I find this very frustrating. Otherwise it works perfectly and I prefer using it to filling out paperwork in the office.Show lessRidgewalkernc2
Not practical
Filled out forms submitted them a new page came up that told me I had to provide my name and email PRIOR to filling out the filled out forms I thought I just sent. Did not refill them out because it is a pretty extensive.Synner22
