This application can help you track your progress and provide support and motivation in between healthcare visits in the following ways: • Delivers medication, medication refill, and appointment reminders that you set up to help you stay on track • Records information you enter such as blood pressure, exercise, mood, etc. and tracks those reported measurements to help you see your trends • Allows you to link external fitness apps like Apple Health to pull in your data around sleep, step count, and more • Enables you to share graphs and trends on your self-reported data with your healthcare professional allowing for more productive conversations during healthcare visits • Provides access to medications, appointments, and self-directed adherence data and organizes it all in one place Care4Today® Connect is not intended to be relied upon as a primary method for scheduling when to take your medications.
