Scoring on the header has been corrected from 4= very severe to just 4 = severe to mirror the PDF example provided. 2. Insomnia was added to Question #6 below as a symptom as in the template 3. Language in Question #14 on the iOS version was corrected as follows: from ‘behavior in interviews’ to the correct verbiage ‘behavior at interview’ Hamilton Depression Scale Updates as follows: 1. All questions now include the intensity nominal scale with an intensity descriptor associated with each answer. 2.
