Our easy-to-use platform is easy to navigate and provides access to a variety of resources so you can actively participate in your care. Not all features listed below are available to all patients: “Today” screen for exploring content and tracking appointments. “Get to Know Me” questions so your care team can know you better. “Memory Wall” for photo uploads to provide more background about who you are. Daily check-in system to let the care team how you’re doing. Invite those important to you to join your support team.
