Comarch HomeHealth 2.0 is an application that is part of a system for remote monitoring of patients, such as chronically ill people and patients after hospitalization. It can be used in one-user (for a single patient) or multi-user (for many patients) mode, such as in nursing homes or by nurses. The system for remote patient monitoring includes the Comarch HomeHealth 2.0 application, available on a smartphone or tablet, and integrated measuring devices tailored to each patient’s needs.
The application guides the user step by step through all measurements, reminds about their execution, and sends the results to the Comarch e-Care 2.0 telemedicine platform, where they are automatically analyzed by the system and then can be accessed by medical personnel. The history of measurements is also saved in the application, so that, depending on the mode, the patient or caregivers and doctors have access to it. The application is very user-friendly and intuitive, designed for seniors, among other groups. With its support, each patient can take measurements independently and correctly.
Application functions:
• Taking measurements and saving the history of results from integrated measuring devices or entering them manually
• Immediate sending to the Comarch e-Care telemedicine platform of the results of health parameter measurements, the ranges of which are determined individually for each patient
• Notification in the Comarch e-Care platform in the case of parameters exceeding the standards

How does it work?
1. The doctor orders remote monitoring of the patient outside the medical facility, adjusts its course and personalizes the ranges of measurement standards
2. The patient performs measurements that are automatically transferred to the application from integrated measuring devices (used pulse oximeters: iChoice OX200, Jumper JPD500F) or are entered into it manually.
3. The results are sent to the Comarch e-Care platform, where they are automatically analyzed by the system and additionally monitored by medical Staff
