COMMpanion is direct primary care, wherever and whenever you need it. From your first appointment, our providers develop a plan custom-made for you, with the goal of optimizing your health. Our dedicated, friendly care team provides:

· Messaging with a nurse
· Prescription refills
· Health coaching

Through COMMpanion, you develop a relationship with a primary care provider (PCP), just like a traditional family practice. The difference is, through our app, you also have access to a team that includes a nurse and a health navigator. Here’s how it works:
· We’ll match you with a PCP for your first appointment.
· Based on your medical history and health concerns, your PCP will build a health care plan for the coming year.
· Your health navigator will reach out to let you know when it’s time to schedule tests, vaccinations and other appointments at one of Penn State Health’s facilities.
· You schedule sick and well visits when you need them, with up to three appointments per month.
· We’ll also notify you when it’s time to refill prescriptions, which can be sent to your favorite pharmacy or even directly to your home.
Have a nonurgent question? You can message your team any time via the COMMpanion app or website and get a response in the same day.
Need a referral or specialist care? Your team can help you with that, too.
Urgent care video visits:
Urgent care video visits with a qualified provider are available all the time, anytime ─ because some things can’t wait until you can get an in-person appointment.
Home health monitoring:
Every family enrolled in COMMpanion receives a home exam kit for monitoring vital signs, like temperature and blood pressure, so your provider can review your vitals and symptoms in real time.
Care whenever you need it:
You can’t always predict when you’ll need medical care. The COMMpanion app makes sure one thing is predictable – care is here whenever you need it, from a consistent team that knows you and how to keep you healthy.
If you are an Apple Health user you will have the option to share important health information with your provider during your video visit. Sharing Apple Health data is optional but allows the provider to see heart rate, weight, and other health information to help them make a more accurate diagnosis.
