Fully editable actions after creation: description, owner, due date, priority, and status. Completed actions can record a completion date; unused actions can be deleted. New graphical cause mapping on each incident. A zoomable tree shows what happened, immediate response, impacts, 4M causes, root cause, actions, and lessons learned. Every impact and 4M branch stays visible, including “no impact” and “no finding.” Cause-mapping insights feed the Assessment Team where they belong: 4M, root causes, actions, impacts, immediate response, findings, and lessons learned. 5-Why and legal text stay with the AI analysis. On the map, “+” to the right adds the next step beside the card; “+” below adds a child underneath. Long texts in “What happened” grow sideways to save height.
