Managing statin muscle pain
61 yo male physician initiated first ever statin with chol 221, Hdl 53, coronary calcium 1 left circ., 31 LAD and no symptoms with regular exercise history. Developed severe tendonopathy/myopathy rather quickly at 11 months. Now suffering 6-7 weeks after d/c therapy from persistent symptoms. More details of course, but where would this physician find contemporary management considerations?

Thank you for the aspirin guide. It does not calculate risk scores or make recommendations for patients that have chronic high RBCs, hemoglobin, and/or platelets; former smokers (which smoking contributes to atherosclerosis); or polycythemia vera. All of which can lead to clot formation, heart attack and/or stroke, especially in a young patient.

Great tool and as a clinician it’s important to remember that no apps can override clinical judgement. Love the Info tab’s detail on the papers that have the evidence based medicine to back up the algorithm are easily accessible and a great resource when looking for details.

I found this guideline actually in my apps, that I had loaded on sometime ago, but found after my patient left the office questioning whether he should be taking a baby aspirin for prevention, and what was my opinion.
This was a patient who is male 42 years old with no significant medical history except historyof slightly high blood pressure, and no contraindication to aspirin under the list that you gave. But I did not see anything to indicate a higher risk of bleeding than in the general population, mainly someone working with cranes, diving equipment, heavy movements of large objects etc. And at risk for serious injury leaving to heavy bleeding in his job.
I told him that has been was controversial in primary prevention but that his wrist was higher than normal because of his job.I think it might be well for you to include a high-risk job such as that or some other type as it's trending against the recommendation for aspirin in primary prevention.
