How BillWitness handles a bill Every charge anchors to a provider, identified by NPI, and tracks across years. Hospitals, physician groups, anesthesia, pathology, radiology, ER physicians, specialists, each becomes a row in the long-term audit. Each itemized line (CPT or HCPCS code, quantity, modifiers, charge) is logged and screened against eight common error patterns: duplicate, upcoding, unbundling, phantom, wrong CPT, quantity error, wrong provider, wrong insurance. Bill, EOB, and patient-payment reconcile three ways. Six discrepancy categories are flagged automatically.
