About
Blade & Bougie is the app an anesthesia resident actually uses on shift: the number you need between cases, the guide you read the night before, and an alarm loud enough to wake you when the pager goes off at 3 AM. The doses, the guides and the crisis checklists live in the app. No account, no sign-up, no server. Open it in a stairwell or a basement OR and it still answers.
Induction agents, pressors in the units they are ordered in, reversal, antibiotics, emergency drugs. Every figure is pinned to a published source — society guidelines and manufacturers' labelling — and checked against every other place the app prints that drug, so the same number never appears two ways. Ask in plain words and a number comes back, not a paragraph.
Main OR — the emergency manual, considerations for medical conditions by system, surgery specific notes, drugs, fluids and equipment, local anesthetic maximums and the calculations. Surgery specific notes is the largest chapter: thirty-eight briefs, one per operation, written the way a resident writes one for themselves the night before. What to get from pharmacy, what lines to put in, what the surgeon will ask for, and the one or two moments in that case that go wrong. Vascular, cardiac and structural, thoracic, airway, cranial, spine, transplant, urology, hepatobiliary and orthopedics. Considerations for medical conditions runs by system — cardiac, pulmonary, endocrine and metabolic, kidney and liver, neurologic and neuromuscular — including a glucose chapter with the insulin charts rather than a flowsheet. ICU — sepsis, ROTEM, balloon pump, Impella and ECMO, transfusion and reversal agents, electrolytes, sedation and delirium, and a daily checklist that ends with FAST HUGS BID. Trauma, Regional and Pain, OB and Peds each have a shelf of their own.
Body weight, allowable blood loss, maintenance fluid and the fasting deficit, drip rates from mcg/min to mL/h, and transfusion. ICU adds the shock pattern, lung-protective tidal volumes and the ARDSnet PEEP table; Peds adds tube size and depth, a weight estimate and maintenance fluid. Ask a question in plain words. The answer is drawn from the app's own library, on the phone itself, with the source named and the textbook chapter to read next. Nothing is sent anywhere.
Your hours, counted without writing anything down. The app learns the places where you spend working hours and records when you arrive and when you leave, then shows the week, the four-week average, your longest stretch and the days you had free — against the published ACGME limits. You name the places, you correct anything that looks wrong, and you copy the numbers wherever your program wants them. It stays on your phone.
Preops - Tap what applies, type what is left, copy the note out. No patient identifiers are needed, nothing is stored, and the form clears itself.
An alarm that breaks through silent mode while you are on call, and is not armed when you are not.
If your department publishes assignments on a web portal, you can sign in to your own account inside the app and it will tell you tomorrow's room, and the faculty you are with, as soon as it is posted. It reads your own schedule and keeps it on your phone.
WHAT IT IS NOT
A study and reference aid, not clinical advice, not a decision-support system, and no substitute for your attending or your institution's protocols. Check every number before it changes anything.
Free. No ads, no accounts, no analytics, no data collection.
