CheckMyVitals Plus is a contemporary method of chronic disease management for Diabetes and Hypertension. It allows patient, providers, dietitians and other clinicians to connect securely as well as create treatment plans for better organization of chronic diseases. It aims to reduce office visits to the doctor office, and create a new method of operation involving patients actively as part of their own treatment plans.
