DCW Nexus

This app is designed to connect caregivers, consumers (their terminology for patients), and care managers to provide a higher level of care. Caregivers are able to do trainings in the app which set their tier of care to 1, 2, or 3 and find consumers to help in the greater Pennsylvania community. Consumers can set their care needs, which are approved by a care manager, request visits from their caregiver, and fill out a checklist of the care they've received.
