PREDICT – Prevention & Early Delirium Identification Carer Toolkit is an evidence-based digital resource designed to support family carers of hospitalised older adults at risk of delirium. Delirium is a sudden change in thinking, awareness, or behaviour that commonly occurs during illness or hospitalisation. It is distressing for patients and carers and is associated with longer hospital stays and poorer outcomes. Many carers want to help but are unsure what signs to look for or how to raise concerns with healthcare staff. PREDICT was co-designed with carers, clinicians, and researchers to provide clear, practical, and accessible guidance that empowers carers to play an active role in delirium prevention and early identification.
