Genentech ePRO Pilot Study v.1.2
The ePRO Pilot study will capture data weekly about your health, well-being, and activity level. By participating in this study, you will be asked to complete the SF-12 and EQ-5D questionnaire through your mobile device. The app will also use sensor data from HealthKit to track your activity levels (i.e. step tracking). In order to participate in this study, you will require a QR code. This app will provide you with a client portal to access and track your activity data and complete your surveys. You may withdraw from the study at any time.
