How to get in the 2nd door after the hospital room there’s a lady on the floor on top of another lady with blood I need help

The puzzles are fine, and I don’t mind advertising popping up BETWEEN chapters, but these ads come out of nowhere just when you think you might have a puzzle solved. It’s almost as if they want to rattle your brain so you’ll have to buy hints.

No logic
Tons of puzzles but no clue as to where to look for answers and limited items to click on. Concept is there but game details are not. That being said unless you want to pay for every clue after the first free one don't download.
