Great app
Been using this now for a few years to take survey inspection notes. It’s made life so much easier. It’s also great that you can create bespoke templates and support is good too.
Great App
I've used this app for reports both in the UK and Canada. It takes a little bit to learn the interface to create a template but the effort is worth it. It creates very good forms which produce great pdf reports.
Perfect
I don't need a flashy, complicated app. Barebones and blue collar. I fill out thousands of delivery tickets a year for my job. Creating a delivery ticket template was easy once you understand what everything is. I have also created templates for Paysheets and fuel purchases. It's really easy once you get the hang of it. This single app has digitized my entire job. I literally never put pen to paper anymore. App has never crashed, data has never been lost.
*UPDATE: this developer is top notch with customer support. If you want a developer who listens to ideas and makes those changes, then go with this guy. I requested a small change to give a couple more options for input and he did the change. Not only that, he then followed up with me a month later to let me know about the changes. Thank you, Clive.

I have found the Forms app, combined with the Unlimited subscription, an invaluable asset for my work. Working in a very fast paced industry, the ability to report issues and record photos fast and accurate is paramount. The app developer has done a very good job creating an app that helps me create templates on the go and issue stunning PDF reports at a touch of a button. My reports have been appreciated far and wide by my peers and I am happy that I managed to get three other members of my team to use my templates for reporting.
A great app, with great features. From what I have experienced on the App Store, this is the best app for overall versatility in reporting, a great asset in any industry that needs to issue reports on a daily basis.

Continued Usage
I know I have previously written a review for this app but feel the need to do so again.
In my previous job I have used it for on-site dinner checks and depot compliance audits.
I am now moving to a job as Head of Site Security and have easily created a daily form that will track all activities via simple drop down lists such as location and action (all secure, door unlocked, security breach etc.
This is a great app.
