App never works
App is the worst. Never saves my info and closes every time I try and re enter the info
App is Impossible. Constantly crashes
Every time I have to use this before a well child check up, I cringe. Signing in is such a pain. I get to the same part every time and before I can even get to the surveys, the app crashes. Iâve had to use this horrible app for a year and a half. Thereâs always some weird work around I have to do to get back in, and I always forget it. This app wonât work on my computer nor my phone. Iâve restarted and redownloaded everything. Still fails me. I really hope my doctorâs office starts using something else for surveys!!!

Terrible. Constantly crashes with no resolution available.
App crashes constantly
Tried to use this app with my doctors office and it keeps giving error messages. So I still had to fill out paperwork and send it in, so whatâs the point of the app? Then the app gets âfixedâ. But even after signing out, uninstalling,reinstalling, updating etc I still canât get the Chadis surveys done for the autism evaluation. Lots of wasted time for a crap app.

Repeatedly receiving an error message just to get the code to my phone to get started and canât find a Support email or anything to diagnose the issue.
