The app keep closing when I try to open it

The app keep closing when I try to open and it keep doing that and I have to check how many days for my quarantine pls can you tell me what’s going on

Does not work

I have not been able to finish the document upload process, there is always a problem, I had to look up the address I would be staying at and had to start over. Once I started over it said I was putting in my birthday wrong when I wasn’t. Very annoying and not cool.
