It also includes disease-specific guidelines to help providers determine eligibility, ways to talk to patients about end of life issues, and information about services that Hospice Austin provides. Referrals submitted through the app will be followed up expeditiously. All patient information received through the referral form will be managed wit HIPPA compliance.
