Hospice Rating

Hospice care is a Medicare-funded service for end-of-life care. While it is funded by Medicare, services are offered by third-party providers. Each provider services specific geographic areas, and selecting a provider can be a difficult and confusing process. This app is designed to assist with that search. Using a map, users can find all Hospice providers which service a given location. The list of hospices is sorted by an overall score, based on data from multiple sources: - Medicare "Quality of Care" measures - Surveys taken by caregivers In addition to rating scores, each provider shows complete contact info and ownership status -- whether it is For-Profit, Non-Profit, or Government owned. Also available are statistics on average caseload and breakdown of patients by symptoms as well as service location -- in home, inpatient facility, and nursing facilities.
