STEP 2. INCIDENT DETAILS
Record the reporter name, department, shift, incident date and time, exact location, and a full written narrative. Required fields are clearly marked so nothing gets skipped. Report date is automatically stamped.
-----
STEP 3. ROOT CAUSE ANALYSIS
Select all contributing factors from 40+ structured options across five categories:
Human Factors
Training Factors
Environmental Factors
Equipment Factors
Administrative Factors
A built-in search makes it fast to find relevant causes. Add a custom entry if needed.
-----
STEP 4. RISK ASSESSMENT
Set Likelihood (1–5) and Severity (1–5) scores with plain-English guidance at each level. A live 5×5 risk matrix shows your position and calculates the overall risk level: Low, Medium, High, or Extreme. Record immediate corrective actions before you share.
