Crashes, wont work on iPhone SE running iOS 16.3
Tapped Report incident, filled out most of form, app flickered and minimized. Tried again and when tapping report incident, the app briefly shows the form then a white screen with header ‘New Incident.’ Then it crashes. Tried several times, same results.

There are a number of bugs when trying to report a positive observation.
The keyboard type changes aren’t helpful and the error reporting doesn’t give the user enough information about what hasn’t been completed for the submission to be competed.
